Worldwide, opioids are the most common cause of drug related deaths.

The number of individuals abusing them has quadrupled in the last 20 years, with an uptick

in heroin use, an even bigger uptick in prescription opioid use, and a large group of folks abusing

both.

Because of their potential for addiction and overdose, opioids are regulated substances

in a lot of countries.

As a class, opioids share one thing in common—they bind to opioid receptors in the brain, spinal

cord, and gastrointestinal tract.

Some are endogenous, meaning that they are produced naturally by the body, like endorphin,

short for endogenous morphine.

But others are exogenous, meaning that they come from the environment, like heroin and

morphine because they come from the opium poppy—a flowering plant that oozes out a

milky white liquid, while others like fentanyl are synthesized in the laboratory.

To understand how opioids work, let's zoom into a region of the brain tissue that has

opioid receptors.

Normally, in the absence of endorphins, inhibitory neurons secrete a neurotransmitter that prevents

nearby neurons from releasing the neurotransmitter dopamine.

Now, let's say someone goes to play a rigorous game of badminton.

Exercise releases endorphins which activate the three major opioid receptors located on

the inhibitory neurons, called the mu, kappa, and delta receptors.

As endorphins binds to these receptors, they block the inhibitory neuron from releasing

neurotransmitters, allowing the dopamine secreting neurons to freely unload dopamine.

The dopamine then gets picked up by a third neuron in the same area.

When dopamine release takes place in pain processing regions of the brain like the thalamus,

brainstem, and spinal cord, the result is feeling less pain.

When dopamine release takes place in reward pathway regions like the ventral tegmental

area, nucleus accumbens, and prefrontal cortex, the result is a calming effect that feels

good.

So that's how it works normally.

But when a powerful exogenous opioid binds to the opioid receptors, the result is a massive

flood of dopamine.

This helps with pain control, but it can also cause an incredible state of euphoria within

the regions of the brain involved in the reward pathway, which is an emotional "high".

Now remember, the purpose of the reward pathway is to train the brain to repeat activities

that cause dopamine-mediated pleasure, so when opioids stimulate this reward pathway,

the brain learns to do that behavior again and again.

With exogenous opioids there are multiple routes to get the drug to the brain.

One way is by ingesting it, but that route is the slowest.

A faster route would be inhalation, because the drug is rapidly absorbed through the lungs.

The fastest route, though, is direct injection of the substance into the blood.

Typically, the faster the exogenous opioid reaches the brain the stronger the relationship

between the behavior and the reward.

Now over time, people that are consistently using a drug, even when taking them as exactly

as prescribed, can develop tolerance which means that with repeated use, they have a

reduced response, and therefore an increased dose is needed to achieve the original response.

At a cellular level, there are two theories that explain why this might happen.

One theory is that opioid receptors might become less sensitive to a drug, and the other

theory is that the neurons may remove opioid receptors from the cell wall in a process

called down-regulation, leaving less receptors available for binding.

In either scenario, tolerance leads to the need for higher and higher doses of a drug,

and often times that tolerance remains for a long time even after tapering from the drug.

Alright, so now let's say that you're at rest, there aren't any drugs or anything

stimulating your reward pathway.

In this situation, your brain keeps your heart rate, blood pressure, and wakefulness in a

normal state, called homeostasis.

Now, let's say that your secret crush sends you a text.

All of a sudden you may feel sweaty and flushed, your heart rate may jump a bit.

You're now above your normal level of homeostasis, because something has changed, right?

But it doesn't stay that way for long, and after the text message, your brain brings

things back down to this baseline.

With repeated drug use, a few things start to happen.

Let's say you take the drug at a specific time and setting, like 3pm in the bedroom,

and, being a depressant, it makes everything go lower, heart rate, blood pressure, and

wakefulness.

Your brain being the smart brain that it is, will pick up on the pattern.

Now, next time, at 3pm in the bedroom, the brain preemptively increases each one, since

it knows that when you take the drug, everything's going to decrease again.

Now, let's say 3pm in the bedroom rolls around, but there's no drug...In that situation,

the brain still increases everything..but the changes aren't countered with the effects

of the drug, and so the person can feel awful, and these are called withdrawal symptoms.

These symptoms can persist to the point where a person may need drugs just to feel normal,

and if that's the case, they are considered to be dependent on that drug.

Now, on the flip side, let's say that you use the drug in an unfamiliar setting, like

at 11pm at a party.

Well in that situation, your body's not ready for the drug and there's no physiologic

"counterbalance" to help offset the effect of the drug.

When that's the case, it can lead to overdose, even on a dose that the person's been normally

taking, and that's often times what happens.

The symptoms of opioid withdrawal include anxiety, shivering, tremors, yawning, body

aches, vomiting, diarrhea, abdominal cramps, runny nose, sneezing, sweating, and an increased

heart rate and blood pressure.

These symptoms can feel really awful, and often prompts people to use opioids again;

a process called negative reinforcement, since you're removing the drug, which causes withdrawal

symptoms which reinforces more drug use to avoid those symptoms.

There is also positive reinforcement from the dopamine-induced euphoria, again leading

to more drug use.

Together this positive and negative reinforcement leads to opioid addiction also known as opioid

use disorder.

The DSM-5 or Diagnostic and Statistical Manual, the 5th edition, defines opioid use disorder

as causing at least two of the following behavior patterns within a year: 1.

Using more opioids or using them for longer than intended.

2.

Being unable to cut down on the use of opioids.

3.

Having opioid use take up a significant amount of time 4.

Having cravings to use opioids.

5.

Having opioid use affect responsibilities at work, school, or home.

6.

Using opioids even if they cause recurrent interpersonal problems 7.

Giving up important activities in order to use opioids.

8.

Using opioids in physically dangerous situations.

9.

Using opioids even if its worsening a physical or psychological problem 10.

Becoming tolerant to the opioids.

And finally 11.

Feeling withdrawal symptoms from opioids.

Having 2 or 3 of these symptoms is considered mild, having 4 or 5 is considered moderate,

and having 6 or more is considered severe.

In addition to ruining a person's life, opioid addiction can also end it in an overdose.

Most often, an opioid overdose causes severe cardiac and respiratory depression, to the

point where a person may have pinpoint pupils and simply stop breathing.

In that situation, the most important thing is to performing rescue breathing, giving

supplemental oxygen, and administering naloxone.

Naloxone is an opioid antagonist that powerfully binds to opioid receptors, and rather than

having a direct effect, it blocks other opioids from binding and activating the receptor.

This works because at any given moment, opioids are binding and unbinding to receptors which

means once an opioid releases its hold on a receptor, the naloxone can simply sneak

in and bind more strongly.

When naloxone given intravenously it can reverse the effects of opioids within minutes, potentially

saving a person's life.

Generally speaking, high doses of strong opioids carry the greatest risk of addiction and death,

and when paired with other substances that can cause respiratory depression, like benzodiazepines,

they're more likely to cause overdose because they can act synergistically to cause respiratory

depression.

It's clear that opioids have strong addictive potential, so their use should be limited

and well defined.

They have a role in controlling acute pain, for example, but the goal should be to use

short-acting opioids at the lowest effective dose for just a few days, and slowly increase

their dose only as needed.

When opioids are used for chronic pain, they should be weaned off as soon as possible,

ideally at a wean rate of 10% of the dose per week.

In general though, chronic pain should be treated with non-opioid approaches.

These include exercise and biofeedback as well as other types of medications such as

acetaminophen and NSAIDs like ibuprofen.

There are also disease-specific treatments, like for migraines use triptans, or for neuropathic

pain use gabapentin, or for joint pain topical pain treatments like capsaicin.

For people with opioid dependence, the most effective treatment is a combination of therapy

with medications.

Specific therapies that work include motivational interviewing, which can be used to understand

why an individual wants to stop using opioids and identify specific barriers to treatment.

Also cognitive-behavioral therapy can help an individual learn about withdrawal, discuss

the thoughts, feelings, and behaviors that lead to opioid usage, and create a plan to

navigate triggers for usage.

Another form of therapy are peer-support programs which use group discussions to help individuals

commit to ending the use of opioids and by holding one another accountable.

In addition, the evidence strongly supports the use of medications, which reliably decrease

cravings and reduce withdrawal symptoms.

Methadone is a full opioid agonist with a long half-life that slowly builds up in the

tissues over time, allowing it to reach a steady-state level within a week.

Steady-state refers to the situation where the overall intake of the drug is more or

less in balance with its elimination, so that the body is exposed to a stable level.

In contrast, buprenorphine is a partial agonist that is sometimes given in combination with

naloxone, forming an agonist/antagonist combination.

Buprenorphine has a ceiling effect, meaning that above a certain dose it does not have

any more of an effect, which reduces the chance of an overdose.

These medications can competitively bind to the opioid receptor without producing the

same euphoria as the opioids they take the place of.

Since they have opioid effects, though, they can cause side effects like constipation,

insomnia, weight gain, hormonal changes, and cardiac arrhythmias.

The good news is that over time, an individual can safely taper their use of opioid treatments

altogether, and this approach is more likely to succeed than simply stopping opioids without

these treatments.

Another medication called naltrexone, can be used for people who are already abstaining

from active opioid use.

Naltrexone is a mu-opioid receptor antagonist that blocks the effects of opioids and helps

maintain abstinence.

Finally, all of these approaches work best when an individual has a strong network of

family and friends offering support.

Opioid use still carries heavy stigma because usage is still tied to a notion of individual

choice and moral failure, even though we now know that opioid use is a consequence of biological,

psychological, and social factors—all of which need to be addressed to maximize the

chances of recovery.

Alright, as a quick recap, opioids stop inhibitory neurons from releasing inhibitory neurotransmitters,

which allows dopamine to flood the ventral tegmental area, nucleus accumbens, and prefrontal

cortex, and causing euphoria.

Long-term use can cause tolerance which is the need for increasing doses to achieve the

same effect, as well as dependence which is the reliance on the opioid to function normally.

The most effective treatment is a combination of therapy and medications, with a lot of

love and support from family and friends.

I was in charge of the music, I turned that mother out.

But what even the most dedicated fans may not realize,

is that the theme song was actually

different for every episode.

- I have no idea how many themes we did for Seinfeld.

I knew that each monologue was gonna be different,

'cause he tells different jokes.

The timing, the length, had to be adjustable

in a way it would still hold water

and still sound like the Seinfeld theme.

Each monologue had its own recording

of the Seinfeld theme to match those timings.

- Old people in Florida, they drive slow and they sit low.

Right?

- [Narrator] It all started with a phone call from Jerry.

- Hello?

- [Man On Phone] Jerry?

- [Narrator] When Jonathan first began to compose the theme,

he knew he wanted to use unique sounds

that didn't drown out Jerry's voice.

He ended up using his own fingers and mouth as instruments.

(finger snapping and mouth popping with rhythm)

- And I had his attention.

- [Jerry] It's like the Declaration of Independence,

who's gonna read that?

- [Narrator] After that Jonathan

spent time watching Jerry on T.V.

He noticed something.

- I watched his HBO special,

and noticed that Jerry has a lyrical delivery to his jokes.

And I put a clock on it, about 110.

And that became the tempo of the Seinfeld theme.

The bass line for Seinfeld,

(piano music)

so simple.

It can start and stop for his jokes,

(piano music)

hold for laughs,

(audience laughter)

and in that way I could architect each piece

of music for each monologue LEGO-style.

(piano music)

And that is how the Seinfeld theme was born.

- [Jerry] Oh my God, this drawer is filled with Froot Loops.

- [Narrator] In the end, Jonathan's music

almost didn't make it onto the show.

- Jerry loved the music, Larry David loved the music.

Everybody on board, except the network.

The notes were: what is this music?

What is that instrument?

Could we not afford real music?

It's distracting, it's annoying.

- [Narrator] But Larry David wasn't having it.

- Larry David, the hero of this story,

liked that it was annoying.

Said, "Don't you dare. Wolff you're done here, get out."

And I left and the music stayed in the picture.

Procrastination: Deadly Spiritual Consequences - Duration: 3:04.

Hi. I'm training to be an end time survivor.

I'm trying to listen to God  so that I can learn how to live in the last days.

A friend shared a dream with me. It inspired me to make this video.

In the dream, people were on a conveyor belt.  At the end of the belt they fell into hell.

People on the belt knew this,  but they planned to jump off at the last minute.

But as they got closer,  it was harder to get off.

The only ones who escaped  were those who jumped off  as soon as they saw the danger.

We here can see that the world  is heading toward disaster.

Great trouble is coming,  along with a new world leader.

People will need to accept the Mark  before they can buy or sell anything.

The Bible says it's better to die  than to accept that mark.

God wants those who believe in Jesus,  to obey Jesus, now, before the Mark arrives.

If God says something today, the Bible says to obey it today.

The longer we delay change,  the harder it is to change.

If you can't at least start today,  you won't do better tomorrow.

Spiritual preparation includes living like Jesus and his disciples lived.

His Father took care of their material needs.

We call it living by faith.  I'm learning to do that now.

That is my lesson from the conveyor belt dream.

It's not easy to change our behavior.  We all have habits that need to change.

I'm praying that God will help me change.  Listening helps me to know where to start.

We may have more time than we think.

But it's better to be early,  than to be too late.

Change needs to happen in our lives  and in our churches.

If others are not helping you to change,  you need to find people who will.

of January but since i had some

technical issues

laughter broke and I don't really have

since this was a bigger compilation

didn't really work out on the phone and

I could it but I was not going to be

really happy with the product so i chose

to wait so I got some computer to edit

the video here we are five days later in

um I'd say that he needs to take out

ISIS because they've been bombing us

I think Trump needs to build a wall in

order to get all the illegal immigrants

out in the main reason is just because

that ruins the economy and we if you

want to help people like immigrants what

you should do is you should help America

first and then go like overseas and help

people over there instead of bringing

them into America because I'll be a

better plan because instead of having to

ruin America in order to make some

people's lives better

you could make America great again and

help some people by just waiting a

couple of years until America is better

and then going overseas

alright well i think first of all he

needs to Humble himself I think it would

be good for him to maybe do some praying

kind of get down on his knees and pray

just to realize that the gravity of his

position and the influence that he has

and the ability for him to really make a

difference a positive difference second

thing I would say is that i think would

be beneficial for him too

maybe be himself in the sense that i

think would be a lot of people pulling

at different angles on him trying to get

to do their job and to the do what they

feel is important i think it'd be good

for him to stick by what he you know is

therefore with the electorate elected

him to do and just do it humbly do it do

it with love

This, right here, all of this, this, and this,

This is my recap.

Especially the years of 2014 to 2016 have been the craziest of my life yet.

I finally graduated from High School,

I turned 19, lost two insanely important people in my life due to cancer,

My parents split up, my father moved away, my mother

was diagnosed with cancer, my girlfriend broke up with

me, I went all in and full time on my Youtube channel,

Got my car and motorcycle licenses, fought my biggest

fears by doing my first skydive, starting to paraglide,

and wreck diving, finally found a manager, became

part of the GoPro family, and was featured on Germany Tv twice.

I broke my ankle and had a tendon ruptured,

had multiple surgeries and I had to learn how to walk again over a period of multiple months.

I started investing money, worked and lived for two

months in China, wore braces for two years

and fell on my mouth the week I got my braces out,

damaged all of my front teeth,

live for one month in a Germany - USA exchange program,

helped multiple presentations at universities though out Germany,

Spoke in front of 15K people, lost 5 GoPro cameras,

cashed my computer twice and lost all the data.

I traveled to 16 Countries in the last two years alone,

spent thousands and thousands of dollars on plane tickets,

but it was worth every single penny.

In that time alone, I produced over 300 Videos,

but this video, this video especially,

I want to say thank you.

Thank you to every single one of you.

Your kind words are the things that kept me going

in the moments when my life seemed like a never ending downward spiral.

So thank you.

were you like as a kid so I thought it'd

be fun to a cruise down memory lane

which is always very hard for me to be

honest I don't remember sheet about my

childhood which is a little concerning

you know because I've done I've done

some research and that's not that's not

healthy

you should should be able to remember

some shit I should probably get

hypnotized figure out I was touched or

not you know i mean i don't remember

anything happening which tells me he

must not have been that good

oh god what a sick joke you know they

always tell you is a good to it's like

no matter what it wasn't your fault

could have been my fault i mean i was a

manipulative little bastard I was like

say a sexy little kid huh

I could wear a diaper no that's all may

weird start my I wasn't like a flaming

kid i don't know why i portrayed myself

so I needed some help with this video so

I called mom which is always fun you

know because I told you yeah she's like

in a different place in her life now

she's been working a lot and she doesn't

have the kind of time and attention that

I'm used to so I you know I call up

yesterday Mike a ma takes a little while

for the phone to connect it's the only

1i have this problem with but so I call

her and i was like my need you to focus

like a deer with a shiny object Chuck to

break out the mail you know it's your

mama does your mother like mine its kind

even notice i talked to your mom it's

kind of like starting a car in the

winter you gotta warm up a little bit

before it's comfortable right

app right so let's start off with the

first one that I kind of enjoyed that

she told me about so apparently we went

to Disney World one-year happiest place

on her sure was the day I went there I

guess I wasn't feeling well I was sick I

wasn't feeling well but you know how

kids are like dogs you know like one

minute they're gentle management drop so

I wasn't feeling well and I guess they

didn't have a stroller with us or

something cause we were traveling right

and and they were out of them at the

theme park so they gave us a wheelchair

so I guess at some point I father's

wheeling me around right and I start

feeling better now my mother is in

shopping most of my childhood she was

either take care of your shopping and I

guess there's like these little

character houses they're only about

eight feet high or something it's like

white picket fences next to a right so

is my father's not paying attention so

I'm sitting there starting to feel

better and I get the idea i'm going to

get out of this wheelchair and somehow I

like I climbed up the fence and up onto

the roof of these things and people were

watching me but i guess the whole reason

I got away with it was because I got out

of the chair and I started yelling it's

a miracle I can walk

oh my god so people think they're

witnessing a child walking for the first

time his kids allowed to do anything and

i kinda I know must have known that

right so I climb up the fence and I am

jumping i'm doing like a little jig on

top of these rooms and crowd is forming

management comes security comes and

can't do anything because the crowd is

clapping finally my mother comes out she

is mortified

she can't say anything to security what

she was saying you guys are we would

have been thrown out of the park by

father's gone he usually we would eat

left like he was not related that was

usually his go-to but just get out of

the situation

oh boy here's another one that was kind

of fun I've always been obsessed with

technology USDA me right now like my

places is a smart is a smartphone

well it's a smart apartment it's

actually really fucking down I don't

understand it so cold in here or it's

screaming hot it's never just

comfortable that doesn't strike me as

smart that's it's an annoying apartment

with a lot of technology anyway um I

always liked technology as a kid except

for nintendo I wasn't a big fan of games

didn't like I'm in fact my brother never

even wanted to play with me because I

was like why we like me is like because

your destructive like a little kid knows

that it was one of the first words he

learned because of me and my lack of

patience you know what happen when the

game wasn't going the way I wanted to I

would take the controller and throw it

was just launch it right into the game

system it'll spark it would stop and

then game over i win

so that's kinda was my go-to and he knew

this

so he was like no I'm not playing with

you but aside from that I had this bed

was obsessed remember those universal

remotes

oh my god i created so much trouble with

those i used to go around with my little

codes I was like I remember this one

time it was super bowl sunday of course

my mother too

us what you do shopping so we're going

around the mall and I we went by a

Hooters and you know how these guys

nothing better than super bowl sunday

but your liquored up guys in hooters

they got all the tvs going right there

cheering it's like doing replays big

parts of the game there's asshole me

what is that a Zenith turn that to

cartoons

oh my god that 1i did it there might get

wherever I went I did in college even

later i'm still obsessed even in college

you know there's tvs in a lot of the

classrooms and stuff mom just walked by

and on about this teacher couldn't

figure it out ever

I loved it speaking of Technology though

you know those little security devices

this was always a fun game

I taught my brother had to do was like

Eric come here watch this so i spent a

lot of time its doors again because my

mother you know to think of it you might

have a problem

oh my you have a problem you shopped way

too much anyway so again we're in stores

and you know how they have the security

devices on that will set off if you take

it outside of the store so I thought of

a really clever way to get the security

devices outside whenever we were in a

toy store i would take another little

wind-up cars i put in a poem

out-of-the-box or whatever wouldn't

steal them but i would then take one of

those security devices put it on top of

the car whenever there was a bunch of

traffic coming in and out of the store i

did my brother watch and i set it off

well stop everyone there searching bags

and meanwhile no one sees there's this

little car now I took the ball with a

secure it

oh my god that was one of my favorite

games now i just want to point out the

use of the games i played as a kid and

now as an adult i play games like makeup

look the maturity

english my age moved as slow as my

maturity level I'd be a hundred and

fifty before I even showed a sign of

major anyway those are my little games

um I just want to thank you by the way i

keep getting more and more outpouring of

support as you guys are seeing these

videos some of your little bit behind

and you're still on i'm happy right now

I feel great but some of you are still

watching me in my world do and you do is

killing me which they're still trying to

do by the way that's not that's not

ended I mean bring it on youtube you

guys have no idea you guys fucked with

the wrong person I've got stuff about

the Hat we are going to attack you know

not like we're not really like that but

if you're gonna feel like you had a bomb
